Community Children's Nursing Service
The Children’s Community Nursing (CCN) service provides a community nursing service for children and young people aged from 0-18 years of age who are registered with an Oxfordshire GP. The service is delivered 365 days a year from 08.00 - 17.00hrs. Respite care is provided for children outside of these hours on a needs assessed basis. For children nearing the end of life a 24/7 service is provided.
The Community Children's nursing service consists of several teams including:
- Community Children's Nurse Team-Specialist Nurse’s visiting families with a child or children whose condition requires nursing advice and support in the community. The aim is to prevent or reduce hospitalisation for children and their families and provide nursing care in the child’s home and other community settings.
- Palliative Care Team : Supporting children and families at the end of their lives.
- Special School NurseTeam- Children’s Nurses working in designated special schools across Oxfordshire. They have responsibility to enable all children to access education regardless of their health needs and to ensure that these health needs are met whilst in education.
- Complex Care Team-The team includes Children's Nurse's, Health Care Assistant’s (HCA), Training Co-ordinator and a Play Specialist. The Nurse’s and HCA’s provide respite care for children with health care needs in community settings and their own homes. The Training Co-ordinator provides specialist training for carers in line with Shared Care Protocols. The Community Play Specialist uses play (including art and music etc) to help children and/or their siblings understand what is happening to them. The play specialist has expertise in supporting children with complex health needs and to children experiencing difficulties coping with medical procedures and treatments that may result in one or more of the following; changes in behaviour, regression, anxiety or fear.
