What we do:
We provide a range of highly skilled staff to provide community based support. This can involve:
- Providing parents with a break from caring
- Personal care/Domicillary care in the young persons own home
- Independence support
- Social coaching/mentoring
- Behaviour Management. (devising, implementing and reviewing)
- Modelling positive interaction
- Supporting with the implementation of parenting plans
- Supporting with the implementation of Child protection Plans
- Providing 1.1 emotional support
- Support to develop individuals communication Skills
- Family support
- Assisting a child/young person to access their wider community
- Increasing confidence and resilience.
- Responding to safeguarding concerns.
Who we offer this service to:
Young people with a diagnosed disability who have had an assessment of need, and are allocated disability services via Childrens Disability panel.
How and when we do it:
We are a needs led service who can provide support 365 days a year if the children/young people and families are assessed and allocated a service, within the resources that are available. The service is provided in the community with highly skilled staff, consisting of support workers, senior support workers, behaviour support workers and flexible response workers.

The service is provided for children and young people up to 18yrs of age and from Oxfordshire.
The child/young person will have had a child and Family assessment carried out by their social worker and be resident in Oxfordshire. Following an assessment, support provided from Children and Family Care Service will have been approved at resource panel.
